College of Physicians and Surgeons of Canada
The College of Family Physicians of Canada produces a series of patient information brochures on many topics including diabetes, exercise, high blood pressure, and healthy living.

The Institute for Healthcare Improvement
The Institute for Healthcare Improvement (IHI) is a not-for-profit organization created to help lead the improvement of health care systems.

Improving Chronic IIllness Care (ICIC)
A national program office of The Robert Wood Johnson Foundation based in the United States at The MacColl Institute for Healthcare Innovation. The model was developed at The MacColl Institute and includes elements of self-management support, delivery system design, decision support and clinical information systems for improving chronic illness care

Healthy Heart Kit
Health Canada's Healthy Heart Kit has been endorsed by the College of Family Physicians of Canada, the Canadian Cardiovascular Society, Canadian Medical Association, the Heart and Stroke Foundations of Canada and the Quebec, and the Canadian Coalition for High Blood Pressure Prevention and Control. The kit includes pamphlets for patients, as well as a Physician's Guide to the Healthy Heart Kit.

Go for Green Prescription Pad
The College of Family Physicians of Canada and Health Canada collaborated on the design and distribution of the Go for Green Prescription. Researchers have found that the effectiveness of physical activity counselling interventions can be improved if family physicians provide written materials for patients. A written 'prescription' for exercise may be particularly helpful.
