Secure Web Site for PractitionersPhysicians can get their probabilistic patient registers and performance reports (as they become available) from the Ministry’s secure web site (login here). If you are a licensed BC physician registered with the Medical Services Plan, you are eligible to access the secure web site. See the Frequently Asked Questions (PDF 62K) about the secure site. Chronic Disease Management ToolkitThe Chronic Disease Management Toolkit is now available to BC physicians. This web-based technology provides a host of patient information management functions needed to optimally provide planned patient chronic care. The technology is especially useful to practices not equipped with an electronic medical record system. For more information, see the Chronic Disease Management Toolkit Fact Sheet (PDF 81K). Request AccessTo request access to the secure web site and Chronic Disease Management Toolkit, please complete and submit the Web Access Request Application and Agreement (PDF 76Kb). For supporting instructions see the Help with Request Access Form. HealthNet Access Services staff can then process your request to become a registered user of the secure web site. Pickup Reports (login here)You may pickup your probabilistic patient registers and performance reports through the secure web site.
